If you are a first-time client, before your scheduled appointment, please submit a health declaration form which can be found at the top of the homepage. The link says" health form " in the top menu. My business is located inside the Longevity Center located at 104 Mills Ave. in Greenville, SC. The parking lot is located behind the building, as well as the entrance. To cancel or reschedule your appointment please contact me 12 hours prior to your appointment. Cancelations or rescheduled appointments less than 12 hours in advance will be charged a fee equal to the scheduled service. Thank you.
